Information: Please read the application & dinner form carefully! All candidates must adhere to the dress code or they will not be able to participate. You must complete the application form 4 - membership number and signature by your council's Financial Secretary is absolutely necessary - and send it in with the application!

Initiation Fee (includes 1 banquet ticket, lapel pin, social baldric, and processing fees)
Lay candidates and permanent deacons $70.00
Priests and members of religious orders $50.00
Banquet tickets for wife, guests, and current members $40.00
Ladies’ luncheon $16.00


REACH YOUR HIGHEST POTENTIAL


The Fourth Degree is a group of outstanding Knights, dedicated to promoting patriotism enlightened by their Catholic Faith. These Knights, distinguished by exemplary deeds for God, for country, and the Order, are united by the Fourth Degree to support religious, civic, and charitable causes.

Fourth Degree members belong to local units called assemblies. Each assembly is involved in a wide range of public patriotic activities.

Each assembly maintains a Fourth Degree Color Corps and Honor Guard. Uniformed members wearing colorful capes, chapeaux and swords, participate in special civic and religious ceremonies. For this reason, the Fourth Degree is often called the visible arm of the Knights of Columbus.

ELIGIBILITY
To qualify for the Fourth Degree, one must:
1. Be over 18 years of age;
2. Be a citizen of the United States;
3. Has been a First Degree member prior to Nov. 30, 2009.
4. Be a Third Degree member in good standing with your Council;
5. Be a practical Catholic in Communion with the Holy See.

EXEMPLIFICATION SCHEDULE & SCHEDULE OF FEES
Please refer to the form listed above.

Each candidate receives one banquet ticket, which is included in his fee. No credit is allowed if the candidate does not attend the banquet. Tickets are not transferable.

All banquet reservations must be made through the Father Joseph C. Tomko Assembly

LADIES LUNCHEON
There will be a Ladies Luncheon beginning promptly at 12:30 PM. See form for the cost.

DRESS The official dress for the Fourth Degree Exemplification is black tuxedo, black bow tie, plain white tuxedo shirt, (NO WING COLLARS), black shoes, and black tuxedo cummerbund (NO VESTS). Religious may wear their clerical garb and members of the armed forces, full time police and firefighting officers may wear their respective dress uniforms. Members shall wear the official dress plus social baldric, lapel pin and current jewel of office. Tuxedos required until 12:00 AM

DEADLINES: All applications for membership in the Fourth Degree and banquet reservations must be made no later than May 30, 2010. Payment in full must accompany all banquet reservations. There will be a $2.00 late charge Admission to any of the events requires a pre-paid reservation. ** No refunds after this date**

Memorial Procedure
Brother knights - the Father Joseph Tomko Assembly provides for a memorial fund in the event of the death of a brother knight. If you know of a member of this assemly who has been recalled to God, please contact the Faithful Comptroller by email, and the Financial Secretary of your council. A $125 Memorial Donation will be sent to the charity of your choice, provided it is in good standing with the Holy Church. We often recommend that this be sent to the Parish, but alternatives will be accepted.
